New Blood Glucose Meter and Test Strips in 2019

Starting January 1, 2019, the blood glucose test strips currently used by members will no longer be covered under the prescription benefit plan. Members will need to change to a new Accu-Chek® meter and test strips. If you continue to use your current test strips, you will pay 100 percent of the cost.
 
What to do next:
 
1. Order a new Accu-Chek® meter. You will receive one at no cost to you. To order, choose one of the following:
• Fill out and mail the enclosed Diabetic Supply Order Form, or
• Call the CVS Caremark® Diabetic Meter Team at 1-877-418-4746.

2. Get a new prescription for Accu-Chek® test strips to fill after January 1, 2019. Choose one of the following:
• Fill out and mail the enclosed Diabetic Supply Order Form,
• Call the CVS Caremark Diabetic Meter Team at 1-877-418-4746,
• Ask your doctor to send a new prescription to your pharmacy, or.
• Sign in to Caremark.com and select “Request a New Prescription” to start delivery by mail.
 
Please note: Prescriptions for Accu-Chek® test strips will not be covered before January 1, 2019.
