Online Diabetes Pilot Programs

Description
Through the Cecelia program, participants will receive remote support on their schedule from a Cecelia Health Certified Diabetes Care & Education Specialist (CDCES) to develop a plan to manage their diabetes. If you are currently using an insulin pump, the Cecelia program may be a good fit for you to learn more.

The diabetes support program offers up to 6 months of health coaching, education, and support to help participants:
• Monitor blood sugar and lower A1c
• Improve time in range (TIR)
• Reduce the number of unhealthy days
• Adjust diabetes devices to ensure optimal usage
• Achieve healthy eating and exercise goals


Description
DayTwo unlocks the science of the gut microbiome to take the guesswork out of managing blood sugar. Registered Dietitians and Health Guides support members in learning how to combine foods or adjust servings to maintain stable glucose levels based on your unique biology – and no food is off-limits! DayTwo's program can accurately predict your blood sugar response before you even take a bite.

Use the DayTwo app to meal plan, chat with your dietitian, and gain access to over 1 million searchable and scannable food, grocery, and restaurant menu items. DayTwo offers up to 12 months of health coaching, education, support, and waived copays.

Waived Copay Benefit for Active Engagement
The member will be eligible for the waived copay benefit up to 1 year as long as they are actively engaged with health coaches and complete a plan to manage diabetes, and complete all HbA1c tests as prescribed. 

If you enrolled in August 2022, your waived copays are
active 9/1/22- 8/31/23.  
If you enrolled in October 2022, your waived copays will be
active 1/1/23- 12/31/23.​

Waived Copay Benefit for Active Engagement

The member will be eligible for the waived copay benefits up to 1 year  as long as they are actively engaged in the program,as defined by their health coach, wear a continuous glucose monitor for a two week time frame quarterly, log meals in a personalized app, and complete a stool sample at the beginning of the program. 

If you enrolled in August 2022, your waived copays are
active 9/1/22- 8/31/23.  
If you enrolled in October 2022, your waived copays will be
active 1/1/23- 12/31/23.

• Active PEIA PPB plan coverage

• Diagnosis of diabetes (Type I, Type II, or Gestational)

• Lab validated A1c test of 5.7 or higher

• Adults or Children Eligible

• Lab validated A1c test of 5.7​ or higher​​

• Must own either iPhone or Android (and be able to use DayTwo app)

• Must be willing to provide stool sample

•​ Members must be 18 years of age or older (no children)​

•​ Must be willing to wear a continuous glucose monitor​


​Lab Testing

HbA1c is required at the beginning of the program and at the end of the program. Member has the choice to upload a recent verified test result from within the last 90 days or to request a home test kit be mailed to their home from Let's Get Checked ( a contracted vendor). Member must complete and return the at home test kit within a timely manner.

*If a member does not upload or take the initial test they will not be enrolled in the program. If the member does not complete their final HbA1c test by uploading or completing a test kit they will lose the remainder of the year of waived copay benefit. 

​​​Lab Testing
It is required that the member complete a gut biome (stool sample) at the beginning of the program

If a member does not upload or take the initial H​bA1c test,​ they will not be enrolled. It is required that the member complete a HbA1c test every quarter for 1 year. The member has the choice to upload a recent (within 90 days) verified result or obtain a lab order from DayTwo and go to a lab and obtain the test. This may involve member cost share (estimated at $10.00 per test)

​Optional:
A member may opt to have DayTwo provide them with an order to have a lipid panel done at the beginning and ending of the program. This may involve member cost share (estimated at $13.00 per test).

Timeline: 
• Program open enrollment window – September 26-October 7, or until max is met
• Coaching begins after confirming HbA1c results

Exclusion Criteria
 Stage 4 or 5 Chronic Kidney Disease
 Pregnant or postpartum within 90 days
 Currently taking fertility treatment
 Has taken antibiotics/antifungals in last 30 days
 Had a bariatric procedure within the last year or getting bariatric procedure within 6 months
 Full colectomy
 Active chemotherapy (IV or oral)
 Radiation treatment for cancer within the last year
