Out-of-State Health Care Providers
The UnitedHealthcare Choice Plus network is your link to out-of-state physicians, facilities and other health care providers.
Effective February 1, 2024, all facilities, campuses, and providers within Memorial Health System is now in-network for PEIA members.
Effective 7/1/2024, Boyd County Kentucky and Washington County Ohio are no longer "carve out counties". Members can use provider's in these counties under the same rules that apply in any other contiguous county. UK King's Daughters is now in-network for PEIA members.
PEIA reserves the right to remove providers from the network, so not all providers listed in the network may be available to you.
