Benefits Guide

The Benefits Guide (Shopper's Guide) is an invaluable tool for all employees and provides information about the plans offered as well as rate tables for premiums for the plan year.

Summary Plan Description (SPD) Plans A, B, & D 

The SPD provides PEIA PPB Plan A, B, & D participants with an easy-to-read description of benefits available through the Plans and instructions on how to use these benefits. The SPD is a summarized version of a portion of PEIA’s Plan Document.​

The SPD for Plan C provides PEIA PPB Plan C participants with an easy-to-read description of benefits available through the Plans and instructions on how to use these benefits. The SPD is a summarized version of a portion of PEIA’s Plan Document.​

Mountaineer Flexible Benefits Booklet 

This booklet provides eligible participants with an easy-to-read description of flexible benefits (dental, vision, hearing, long and short-term disability, group legal insurance, flexible spending accounts, health savings accounts and limited healthcare FSA) that may be available to them.​ To view additional forms from Mountaineer Flexible Benefits, please view the Forms & Downloads page. 


Medicare Retirees

Medicare Benefits Guide 

The Medicare Benefits Guide is an invaluable tool for Medicare and provides information about the plans offered as well as rate tables for premiums for the plan year.

Medicare Advantage and Prescription Drug Plan Booklet 

This booklet describes the benefits provided for certain PEIA-covered, Medicare-eligible retirees. PEIA has a contract with Humana, Inc. to provide health and prescription drug benefits to Medicare-eligible retired employees and Medicare-eligible dependents of retired employees through the Medicare Advantage and Prescription Drug Plan. These benefits are for members whose primary insurance is Medicare. To review the Annual Notice of Changes and Evidence of Coverage, please visit Humana's website. 

Special Medicare Plan Booklet 

This booklet describes the benefits provided for PEIA insureds who have Medicare as their primary insurance coverage, but are not enrolled in the Humana/PEIA Medicare Advantage and Prescription Drug Plan. Enrollment in the Special Medicare Plan is generally for no more than one calendar year.
